Bright Star Care is looking for a compassionate, skilled, and dependable Home Health Registered Nurse (RN) to join our growing clinical team serving Tyler, TX, and surrounding communities
.
As a Home Health RN, you’ll have the opportunity to provide meaningful, one-on-one care to patients in the comfort of their own homes. This role is ideal for a nurse who values autonomy, clinical variety, relationship-building, and making a direct impact on patients and their families.
You’ll provide skilled nursing care, complete comprehensive assessments, manage complex clinical needs, educate patients and caregivers, and work closely with physicians and the interdisciplinary care team to help patients achieve the best possible outcomes.
What You’ll Do Clinical Care & Patient Assessment- Complete Start of Care (SOC), Recertification, Resumption of Care, Evaluation, and Discharge visits
- Perform comprehensive nursing assessments and develop individualized plans of care
- Monitor patient conditions, identify changes in health status, and communicate concerns appropriately
- Support patients recovering from hospitalization, managing chronic conditions, or requiring ongoing skilled nursing care
- Promote patient safety, independence, and quality of life in the home setting
- Provide a variety of skilled nursing procedures based on patient needs and physician orders
- Perform wound assessments and wound care
, including monitoring healing progress and recognizing signs of infection or complications - Provide catheter care and management
, including urinary catheter care and patient/caregiver education - Perform laboratory specimen collection and lab draws as ordered
- Administer medications and monitor effectiveness and potential adverse reactions
- Assist with disease management and post-hospital recovery
- Provide infection prevention and control education
- Monitor medical conditions and elevate changes in condition appropriately
- Coordinate care with physicians, therapists, patients, caregivers, and other healthcare professionals
- Collaborate with interdisciplinary team members to promote continuity of care
- Participate in case conferences and care plan updates
- Communicate patient progress, concerns, and changes in condition to the appropriate members of the care team
- Assess patients for appropriate home health services
- Identify needs for Physical Therapy (PT), Occupational Therapy (OT), and Speech Therapy (ST)
- Coordinate therapy evaluations and ongoing services as appropriate
Help patients and their caregivers feel confident managing care at home by providing education on:
- Disease processes and symptom management
- Medication management
- Wound care
- Catheter care
- Infection prevention
- Fall prevention and home safety
- Self-management strategies
- Medical equipment and device use
- When to report changes in condition
- Complete accurate and timely electronic documentation
- Maintain thorough clinical records and care plans
- Follow home health regulations, agency policies, and professional nursing standards
- Maintain HIPAA compliance and patient confidentiality
- Ensure care is provided according to physician orders and the established plan of care
Required:
- Current Texas Registered Nurse (RN) license in good standing
- Current CPR/BLS certification
- Minimum 1 year of nursing experience
- Strong clinical assessment and critical-thinking skills
- Excellent communication and organizational skills
- Valid driver's license
- Reliable transportation and current auto insurance
- Previous home health experience
- Experience completing Start of Care (SOC) assessments
