Job Overview

Licensed Practical Nurse (LPN) – PRN position. Provide person‑centered care to residents in assisted living, memory support, and skilled nursing. Build relationships with residents, families, and interdisciplinary team. Perform assessment, care planning, daily support, documentation, and emergency response.

• Assess residents’ needs and develop individualized care plans.
• Deliver high‑quality direct care and assist with activities of daily living.
• Document care accurately and maintain compliance with federal, state, and organizational standards.
• Collaborate with CNAs, physicians, and other staff to ensure continuity of care.
• Respond confidently in emergency situations and follow protocols.
• Promote a safe, respectful, and engaging environment for all residents.

• Current State Nursing License.
• CPR/AED certification.
• Experience in assisted living, memory support, or skilled nursing preferred.
• Strong communication, teamwork, and problem‑solving skills.
• Commitment to continuous improvement and excellence in care.

• PRN (as needed) schedule with flexibility for weekends and holidays.
• Work is on a hospital trust basis.
