Position: PACT Licensed Vocational Nurse (Ukiah)

Summary

The Patient Aligned Care Team (PACT) Licensed Vocational Nurse (LVN) at the Ukiah VA Outpatient Clinic provides direct patient care and clinical support to Veterans as part of the VA's team‑based primary care model. The LVN delivers safe, compassionate, and patient‑centered nursing care while supporting the coordination of services within the PACT model. The LVN plays an important role in promoting continuity of care, improving patient outcomes, and ensuring Veterans receive timely access.

Qualifications

Applicants pending the completion of educational or certification/licensure requirements may be referred and tentatively selected but may not be hired until all requirements are met.

Basic Requirements
  • United States Citizenship; non‑citizens may only be appointed when it is not possible to recruit qualified citizens per VA Policy.
  • English Language Proficiency; proficiency in basic written and spoken English is required per 38 U.S.C. § 7402(d).
  • Education:

    Graduate of an approved practical or vocational nursing school or equivalent, including military service education accepted by the licensing body.
  • Licensure:
    Active, full, current, and unrestricted licensure as an LPN/LVN in a U.S. state, territory, commonwealth, or the District of Columbia.
  • Grandfathering Provision:
    May qualify per the VA Qualification Standard for current VHA employees who meet criteria.
Grade Determinations
  • GS‑3:
    None beyond the basic requirements.
  • GS‑4:
    Six months of qualifying experience as an LPN/LVN or comparable education and experience, plus the ability to perform duty level of GS‑3.
  • GS‑5:
    One year of qualifying experience at GS‑4 level with demonstrated ability to provide a full range of practical nursing care.
  • GS‑6:
    One year of qualifying experience at GS‑5 level with technical proficiency in initiating, performing, and completing assigned duties.
Preferred Experience
  • Two years of outpatient nursing experience.
Salary

Salary ranges by grade: GS‑3: $51,058–$66,381 annually; GS‑4: $57,313–$74,511 annually; GS‑5: $64,124–$83,362 annually; GS‑6: $69,153–$89,898 annually.

Physical Requirements
  • Must serve adult patients age 18 to 65 and older.
  • Visual acuity correctable to 20/40; hearing clear speech correctable to 20 dB; manual dexterity for procedures and computer use.
  • Ability to stand, walk, stoop, sit, bend, pull, push, and transfer patients and objects for extended periods.
  • May be exposed to infectious patients and required to wear protective clothing in isolation or during invasive procedures.
  • May encounter combative patients due to delirium, dementia, or psychiatric disorders.
  • Must remain mature, flexible, and able to handle stressful situations and shift priorities.
Duties
  • Obtain and document patient vital signs, pain assessment, and other vital data.
  • Perform patient intake and prepare Veterans for provider examinations.
  • Assist providers with physical exams, procedures, and treatments.
  • Administer medications, immunizations, and injections per provider orders and established protocols.
  • Perform clinical procedures such as wound care, dressing changes, electrocardiograms (EKGs), specimen collection, and point‑of‑care testing.
  • Monitor patient condition and report significant changes to the RN or provider.
  • Maintain infection control practices and a clean clinical environment.
  • Coordinate patient care activities within the PACT team.
  • Schedule follow‑up appointments, diagnostic tests, and specialty referrals as directed.
  • Manage patient panels, monitoring preventive care reminders and chronic disease management.
  • Participate in telephone triage and relay patient concerns to the appropriate provider or RN.
  • Track laboratory results and ensure appropriate follow‑up.
  • Educate Veterans and family members on medications, treatments, and disease management.
  • Reinforce provider instructions and encourage patient participation in care plans.
  • Promote preventive health measures such as vaccinations, screenings, and healthy lifestyle choices.
  • Accurately document patient care and communications in the electronic health record per VA policy.
