Key Job Duties And Responsibilities
The Licensed Practical Nurse provides direct nursing care to the residents in accordance with federal, state, and local standards, guidelines, and regulations, and ensures highest quality care.
Mandated responsibilities include:
- Comply with the facility’s personnel, safety, and corporate policies and procedures.
- Comply with the nursing department’s policies and procedures.
- Report witnessed or reported allegations of resident abuse/neglect immediately to the Administrator, Director of Nursing or Unit Manager.
- Treat residents, family members, visitors, and team members with dignity and respect.
- Maintain the confidentiality of all resident and family information.
- Report to work on time and as scheduled.
- Assist in the admission and discharge of residents.
- Assist residents in daily hygiene care including bathing, oral, ear, eye, facial, hair and nail care.
- Keep residents dry by changing clothing and linen when necessary.
- Assist residents in bowel and bladder functions; collect specimens when required.
- Assist in transporting residents to appointments, activities, and social programs.
- Assist with lifting, turning, moving, positioning, and transporting residents into and out of beds, chairs, bathtubs, wheelchairs, and lifts.
- Report and document any change observed in a resident’s condition or behavior to the Charge Nurse.
- Assist in the care of dying residents; provide post‑mortem care.
- Administer professional services such as catheterization, tube feedings, suction, dressing changes, packs, colostomy, drainage bags, blood collection, etc., as required.
- Perform lab tests, blood pressures, and other routine monitoring as required per the resident care plan; admit, transfer, and discharge residents as necessary.
- Complete accident/incident reports and other administrative duties such as medical forms, evaluations, charting, etc.
- Submit accident/incident reports to the Director and follow established procedures; report discrepancies in physician’s orders or charting errors to the Nurse Supervisor.
- Develop and maintain a good working rapport with inter‑departmental personnel to ensure nursing services and activities meet resident needs.
- Review complaints and grievances and report to the Nurse Supervisor as required.
- Receive and give the nursing report upon shift start and end.
- Report occupational exposures to blood, body fluids, infectious materials, and hazardous chemicals per facility policies.
- Monitor assigned personnel to ensure compliance with safety regulations.
- Recommend equipment and supply needs to the Nurse Supervisor.
- Review care plans daily to ensure appropriate care is rendered.
- Oversee resident needs, staff, and operations as needed.

- Regular worksite attendance.
- Valid professional license (LPN) in good standing with the State.
- Valid CPR certification.
- Basic computer skills and proficiency in reading, writing, speaking, and understanding English.
- Ability to work beyond normal hours and on weekends and holidays when necessary.
- Ability to assist in the evacuation of residents during emergency situations.
- Ability to perform the essential position functions with or without reasonable accommodation.
- Use of transfer/gait belt or assistance when lifting or carrying objects in excess of 25 pounds.
