Promotes and restores patients' health by completing the nursing process; collaborating with physicians and multidisciplinary team members; providing physical and psychological support to patients, friends, and families; supervising assigned team members. The LPN will report directly to the Director of Nursing.
Job Responsibilities- Explaining services by performing community outreach. He or she will provide education to the clients, hospital staff, physicians, community stakeholders on Home Care Services and the admission process.
- Promotes and restores patients' health by completing the nursing process; collaborating with physicians and multidisciplinary team members; providing physical and psychological support to patients, friends, and families; supervising assigned team members.
- Confer with the RN supervisor regarding needed changes in the Plan of Treatment/Care.
- Provide nursing assessments and patient education.
- Treat wounds, change wound dressings, give injections, administer enema, and diabetic foot checks.
- Collect specimens from patients.
- Administer medications and set up medication dispensers.
- Measure and record vital signs.
