Department of Special Services Transition Coordinator 2026 - 2027 School Year

The Township of Union Public Schools is seeking a dedicated, knowledgeable, and qualified Transition Coordinator

REQUIRED:
  • NJDOE Certification

    Required:

    Teacher of Students with Disabilities (TOSD) OR Teacher of the Handicapped certification (TOH) required.
  • Demonstrated experience working with students with disabilities in a school or educational setting.
  • Knowledge of special education law and regulations, including N.J.A.C. 6A:14, IDEA, and NJDOE transition requirements.
  • Knowledge of transition planning and services for students with disabilities.
  • Knowledge of postsecondary education and training, employment, vocational services, independent living, and community resources.
  • Demonstrated ability to collaborate effectively with students, families, educators, administrators, community agencies, employers, and other stakeholders.
  • Strong organizational, communication, interpersonal, and problem-solving skills.
  • Ability to maintain accurate records and documentation and manage multiple responsibilities and deadlines.
PERFORMANCE RESPONSIBILITIES:
  • Coordinate and support the district's transition planning and transition services for students with disabilities.
  • Collaborate with case managers and IEP teams to support compliance with transition requirements under N.J.A.C. 6A:14.
  • Assist staff in developing meaningful, individualized transition plans based on each student's strengths, preferences, interests, needs, and age-appropriate transition assessments.
  • Attend conferences with parents/guardians, Child Study Team members and instructional staff, as necessary, and review and update students’ transitional goals, objectives, and activities during IEP conferences.
  • Support the development of measurable postsecondary goals related to education/training, employment, and, when appropriate, independent living.
  • Coordinate transition-related assessments, activities, services, and experiences.
  • Develop and maintain relationships with community agencies, employers, postsecondary institutions, vocational programs, and adult service providers.
  • Facilitate connections between students and appropriate adult agencies and community resources.
  • Collaborate with the New Jersey Division of Vocational Rehabilitation Services (DVRS), New Jersey Commission for the Blind and Visually Impaired (CBVI), and other agencies, as appropriate.
  • Assist eligible students in accessing Pre-Employment Transition Services (Pre-ETS), including job exploration counseling, postsecondary education counseling, work-based learning, workplace readiness, and self-advocacy opportunities.
  • Develop, coordinate, and expand work-based learning, career exploration, internship, employment, and community-based experiences for students with disabilities.
  • Establish and maintain partnerships with local businesses and community organizations to increase meaningful employment and career-development opportunities.
  • Coordinate student participation in work-based learning experiences and ensure that applicable NJDOE requirements for WBL placement and supervision are met.
  • Collaborate with school personnel to ensure that transition activities are appropriately reflected in students' IEPs and implemented as planned.
  • Support students and families in exploring postsecondary education, vocational training, employment, adult services, and independent living options.
  • Provide information and professional development to staff regarding effective transition planning, assessment, WBL, community-based instruction, and postsecondary opportunities.
  • Maintain accurate records and documentation related to transition activities, agency referrals, WBL experiences, community partnerships, and student participation.
  • Monitor transition programming and assist in evaluating the effectiveness of transition services and student outcomes.
  • Assist with transition-related district initiatives, programs, events, and activities.
  • Perform other duties as assigned by the Director of Special Services or Superintendent.
  • SALARY RANGE:

    $70,500 - $100,904 in accordance with the UTEA Agreement based on education and experience.
