We are seeking an experienced and dedicated professional to join our healthcare team as a Chronic Care Manager Supervisor. The ideal candidate will have a minimum of 3 years of hands-on experience in chronic care management, coupled with at least 1 year of supervisory experience. In this role, you will be responsible for overseeing and guiding a team of chronic care managers to ensure the delivery of high-quality, patient-centered care for individuals with chronic health conditions.

Responsibilities:

Supervision and Leadership:
- Provide leadership and guidance to a team of chronic care managers.
- Conduct regular team meetings to facilitate open communication, share insights, and address challenges.
- Foster a culture of collaboration, continuous learning, and excellence within the team.
Chronic Care Management:
- Utilize your expertise in chronic care management to support the development and implementation of care plans.
- Oversee the development and implementation of individualized care plans for patients with chronic conditions.
- Ensure that care plans are tailored to meet the unique needs of individuals with chronic health conditions.
- Proactively identify and address barriers to effective care coordination.
- Stay abreast of the latest developments in chronic care management and incorporate best practices into team processes.
Quality Assurance:
- Monitor and evaluate the quality of care provided by the chronic care management team.
- Implement and oversee quality improvement initiatives to enhance patient outcomes.
- Ensure compliance with relevant healthcare regulations and standards.
Team Training and Development:
- Conduct ongoing training sessions to keep the team updated on the latest advancements in chronic care management.
- Identify and address training needs to enhance the skills and knowledge of team members.
Collaboration:
- Collaborate with other healthcare professionals, including physicians, nurses, and social workers, to ensure comprehensive and coordinated care.
- Establish and maintain effective communication channels with internal and external stakeholders.
Data Management:
- Oversee the collection and analysis of data related to chronic care outcomes.
- Use data-driven insights to identify areas for improvement and implement changes as needed.

Qualifications:

Bachelor's degree in nursing, healthcare management, or a related field;
Master's degree preferred.
Minimum of 3 years of direct experience in chronic care management (CCM), with a proven track record of positive patient outcomes.
At least 1 year of supervisory or managerial experience in a healthcare setting (CCM supervisor preferred).
Strong understanding of chronic health conditions and evidence-based interventions.
Excellent communication, leadership, and interpersonal skills.
In-depth knowledge of chronic care management protocols, best practices, and emerging trends.
