Registered Nurse Case Manager – LTSS – Field in Union County, NJ

Position Summary: We are seeking a dedicated Remote/Field based Case Manager (RN) to join our team, serving members in Union County, NJ. This role involves 50-75% field travel and follows a Monday-Friday, 8:00 AM-5:00 PM EST schedule.

The Case Manager will develop, implement, and support health service strategies, policies, and programs that ensure high-quality, cost-effective care for our members. These strategies encompass utilization management, quality improvement, network coordination, and clinical policy development. The position requires strong clinical judgment, critical thinking, and the ability to coordinate physical, behavioral, and psychosocial services.

• Must reside in Union County
• 3+ years clinical practice experience (e.g., hospital setting, alternative care setting such as home health or ambulatory care)
• Active and Unrestricted RN license in NJ
• Must possess reliable transportation and be willing and able to travel up to 75% of the time. Mileage is reimbursed by the company expense reimbursement policy.
• Successful completion of the NJ Choice certification (score of 80% or higher, along with completion of State training modules and field mentoring component).

• LTSS experience
• Case management and/or discharge planning experience
• Managed Care experience
• Medicaid and Medicare experience
• Experience using computers, keyboard, mouse, multi-system navigation, and MS Office Suite applications (Outlook, Word, Excel, SharePoint, Teams).

• Minimum of Associate degree or Diploma in Nursing

Pay Range: $66,575.00 - $ (base hourly rate or base annual full-time salary; actual offer depends on experience, education, geography and other factors. Eligible for bonus, commission or short-term incentive program.)
