Position: Registered Nurse, Hospice / Home Care Coordinator, Per Diem, Days, Northern / Central NJ

Responsible for working closely with hospitals and other facilities to coordinate continued patient care with hospice home healthcare services after discharge. Assures quality customer service to patients, families, referral sources and co-workers. Serves as a consultant, educator, and customer service specialist to referral sources.

Responsibilities
  • Develop initial plan of care for a Hospice admission to Home Hospice or to a General Inpatient bed.
  • Coordinate with patients, families, physicians, and discharge planning personnel, by obtaining complete clinical information for the hospice admission.
  • Contact the Hospice Manager as needed to review the appropriateness of General Inpatient Hospice.
  • Communicate the levels of care for Hospice to patients and families, and have the patient admitted to the appropriate level of care.
  • Obtain an initial Hospice order from the referring physician and contact one of the Hospice Medical Directors for certification of terminal illness, for admission to General Inpatient Hospice.
  • Maintain current knowledge of clinical issues, regulatory and reimbursement standards, insurance contracts and negotiated rate amounts.
  • Perform other related duties as assigned.
  • Qualifications Required
    • Associate's degree in Nursing
    • License for Registered Nurse
    • Clean driving record
    • BLS certification
    Preferred
    • Completed apprenticeship or experience in a similar role
