Palliative Care Nurse, Nurse Practitioner
Responsible for working closely with hospitals and other facilities to coordinate continued patient care with hospice home healthcare services after discharge. Assures quality customer service to patients, families, referral sources and co-workers. Serves as a consultant, educator, and customer service specialist to referral sources.
Responsibilities- Associate's degree in Nursing
- License for Registered Nurse
- Clean driving record
- BLS certification
- Completed apprenticeship or experience in a similar role
