We are seeking an experienced Nurse Practitioner to provide primary care to homebound and palliative care patients in the Union County, NJ region. You will apply medical techniques and principles under the supervision of a physician and care for a diverse set of patients with chronic diseases.
What You’ll Do- Provide high‑quality care to primary and palliative patients in and around Union County, NJ
- Perform physical examinations, obtain and update medical histories, and develop care plans
- Communicate effectively with office staff and clinical operations across internal platforms
- Deliver patient‑centered care to a panel of patients
- Attend quarterly medical staff meetings
- Develop diagnoses and treatment plans
- Build trust and consult with patients, families, and caregivers during visits
- Order diagnostic testing as medically necessary
- Prescribe medications, supplies, and appropriate interventions
- Board certification and a valid NP or PA credential in New Jersey
- Strong clinical skills with the ability to assess, diagnose, and treat confidently
- Positive demeanor and strong interpersonal skills
- Confidence in problem‑solving and independent operation
- 1–2 years of primary‑care experience with geriatric patients (hospice experience highly preferred but not required)
- A flexible schedule to accommodate patient availability
- Autonomy in clinical decision‑making
- Support from an in‑house clinical team for requests or orders

- $130k+ (depending on experience and other incentives)
