$20,000 Sign‑On Bonus for Full‑Time Admissions RNs Only

• Complete 1–3 Start of Care (SOC) visits each day across Monmouth, Middlesex, Somerset, and Union Counties.
• Conduct comprehensive patient assessments and accurately complete OASIS documentation.
• Develop individualized plans of care in collaboration with physicians and the interdisciplinary care team.
• Deliver skilled nursing care, including medication administration, wound care, treatments, and patient education.
• Educate patients and caregivers on disease management, medications, and home safety.
• Monitor patient conditions and communicate changes appropriately.
• Ensure documentation is accurate, timely, and compliant with regulatory standards.

• Current Registered Nurse (RN) license in the State of New Jersey.
• Minimum of one year of nursing experience (home health experience preferred).
• Experience completing OASIS documentation and Start of Care visits strongly preferred.
• Comfortable using electronic medical records (EMR);
  Homecare Homebase (HCHB) experience is a plus.
• Excellent assessment, communication, and time management skills.
