Pay
$100,000 to $133,000 per year approximately. The clinician will be paid $55 per point, with an expectation of 30 productivity points completed per week. Additional pay can be earned by exceeding 30 points per week.

Position Requirements- Comprehensive assessments in the patient’s home, including skilled documentation through Homecare Homebase.
- Evaluate the patient and home health environment; perform clinical documentation and visit notes; develop care plans; communicate with physicians, care team members, patients, and families.
- Comfortable treating wounds and performing infusions.
- Serve as the patient’s advocate and maintain ongoing communication with physicians, referral sources, caregivers, and facility management to facilitate coordinated care.
- Administer medications and treatments as prescribed by physicians. Provide direct patient care including infusion therapy, wound care, teaching, and disease management.
- Provide direct supervision of nursing clinical care team members (LVN’s & HHA’s).
- Participate in regular case conferences and in‑services.
- Strong organizational skills and time management.
- Ability to multitask and meet timelines.
- Professional appearance and effective communication skills.
- Current Texas Registered Nurse (RN) license.
- Three years of clinical nursing experience.
- Current Texas driver’s license.
- CPR certification required.
- Experience with healthcare software (Homecare Homebase preferred).
- Competency to carry out any skilled procedure that the physician orders within the scope of the Nurse Practice Act.
