Job Summary and DescriptionProvide case management as a best practice ensuring excellence in service and documentation in all phases including screening, assessment, stratifying risk, planning, implementation, follow-up, transitioning, communicating post-transition, and evaluation. Case managers participate as an interdisciplinary team member in both staffing and consultation to ensure the continuum of care for essential needs as well as mental health.
- Two (2) years of directly related social work experience including case management.
Education and Experience:
- Master’s degree in social work or LMSW.
- Five years of experience in social work.
