Employment Type: Full time Shift: Day Shift Description: The Pre Liver Transplant Coordinator assists in the pre-transplant phase of the patients evaluation. Reviews diagnostic tests, laboratory work, and other pertinent information in clinic and before medical review board. Coordinates care of the patient, triages calls, and assists in medical and medication management.
Candidate preferred to have at least one year transplant coordinator experience
A transplant coordinator assists in the pre-transplant evaluation as well as the post-op follow up of transplanted patients as well as their donors. He/she can provide education to the living donor and recipient regarding the transplant process. They are also on-call for organ procurement/donor offers as well as for patient concerns.
Assists in managing the clinical care of the patient or donor in the pre and post-transplant phase of evaluation for organ transplant. Reviews diagnostic tests, laboratory work, and other pertinent information in clinic and before medical review board. Coordinates care of the patient or donor pre and post-transplant, triages calls, and assists in medical and medication management.
Educates the patient and/or donor on all aspects of the transplant process.
Prepares and submits all information to UNOS, CMS, JCAHO, insurance companies, etc. related to transplant service inquiries.
Performs administrative functions as preparation of QI reports, attending meetings and conferences, etc. as needed.
Position RequirementsMinimum Education: Bachelor's Degree (Nursing)
Minimum Experience: 2 years of previous RN experience.
Preferred: Previous RN Coord experience.
Required Licensure/
Certifications:
Basic Life Support (CPR). Nursing - Current Registered Nurse license State of Illinois.
Salary Range: $48.14- $63.75 per hour. Actual compensation will fall within the range but may vary based on factors such as experience, qualifications, education, location, licensure, certification requirements, and comparisons to colleagues in similar roles.
