Speech‑Language Pathologist (CCC‑SLP) – Full Time

• Effectively screen/evaluate patients with communication, cognitive, or swallowing disorders and develop appropriate plans of care following all regulatory and clinical practice standards.
• Provide rehabilitative, skilled and medically necessary treatment intervention to patients with communication, cognitive and/or swallowing disorders per regulatory and clinical practice requirements.
• Evaluate treatment outcomes for effectiveness; modify services in relation to patient evaluation and make referrals as required.
• Provide consultation and counseling to patients, families, caregivers and other service providers related to speech disorders.
• Provide comprehensive discharge summaries of services per regulatory requirements.
• Document all regulatory and clinically required information in the electronic documentation system and maintain patient records according to company policy.

• Licensed Speech‑Language Pathologist.
• Certificate of Clinical Competence (CCC‑SLP) required.
