OverviewEvaluates and treats patients of all ages, modifying plan of care as appropriate. Serves as a role model in delivery of professional services and as a clinical resource for staff and students.
Responsibilities- Demonstrates ability to perform patient transfers with assistance as needed from bed, chair and floor, ensuring safe body mechanics to reduce injury risk.
- Treats patients and colleagues with dignity, respect, empathy, and full engagement.
- Designs and adjusts a plan of care that includes goals, treatment, outcomes, and discharge planning based on patient response and evidence.
- Educates patients, families, caregivers, students, and other health care providers using effective teaching methods.
- Evaluates clinical findings to determine physical therapy diagnoses and outcomes of care.
- Performs examinations selecting reliable, valid methods tailored to chief complaint, screening results, and history.
- Produces documentation in accordance with Clinical Process Policy (CTSCP
67). - Screen patients to determine effectiveness and necessity of therapy services when appropriate.
- Uses support personnel in accordance with legal and ethical standards, monitoring and providing feedback on their care.
- Completes annual education for DNV Hip and Knee Replacement Program requirements when applicable.
- Implements intervention plans that are functional and responsive to patient behavior and performance.
- Utilizes standardized outcome measures to ensure accurate reflection of functional performance.
- Collaborates with patient, family, and significant others throughout rehabilitation, providing education and family training as necessary.
- Provides supervision to PTA and rehabilitation technicians in compliance with regulatory agencies.
- Communicates patient status, activity and transfer recommendations, and discharge disposition clearly to the medical team.
- Bachelor's Degree in Physical Therapy.
- Licensed Physical Therapist (PT) – Illinois Department of Financial and Professional Regulation (IDFPR).
- Basic Life Support (BLS) certification within 30 days – American Heart Association (AHA).
- Attends and completes all required continuing education regarding acute stroke patient care.

Salary RangeThe compensation range for this position is $36.74 per hour – $63.19 per hour.
