The Home Care Physical Therapist evaluates and develops treatment plans for patients of all ages, modifying the plan of care as appropriate. Serves as a role model in the delivery of professional services and as a clinical resource for staff and students. Hours may vary depending upon census and program need.

Qualifications
  • Bachelor's Degree in Physical Therapy
  • Licensed Physical Therapist (PT) – Illinois Department of Financial and Professional Regulation (IDFPR)
  • Basic Life Support (BLS) within 30 days – American Heart Association (AHA)
  • Driver’s License – varies
  • Proof of Auto Insurance – varies
  • Minimum 1+ years of experience

    Skills:

    Ability to work as a member of an interdisciplinary team; drives reliable transportation and maintains state required insurance and valid Illinois Driver’s License.

Continuing education requirements for this position will include content assigned by the organization to comply with DNV Hip and Knee Replacement Certification requirements. This content will only apply to those directly assigned to the total joint replacement practices. This requirement is subject to change pending updates to DNV Hip and Knee Replacement Certification standards.

Responsibilities
  • Document detailed patient visit information, enabling following staff to determine exactly what and how care was rendered with a clear plan for the next contact.
  • Educate and document discharge planning prior to discharge, including communication with physician, insurance source, outpatient visits and community resources; plan and arrange these if applicable, and provide patient with discharge documentation.
  • Update goals promptly, ensure goals are closed as met or changed as patient condition progresses.
  • Collaboratively communicate and initiate case conferencing as necessary and document interactions with other disciplines, MD, care coordinator, insurers, etc.
  • Communicate requirements for collaborative team and caregivers; assign with scheduler; prepare written instructions for the assistant and supervise the assistant in the patient home.
  • Administer treatment involving application of physical agents such as heat, cold, light, water and electricity or traction and massage therapy.
  • Evaluate, fit and adjust prosthetic and orthotic devices and recommend modifications to the orthotist/prosthetist.
  • Plan and administer prescribed physical therapy treatment programs to restore function, relieve pain and prevent disability, injury or loss of body part.
  • Perform patient assessment tests, measurements and evaluations (e.g., range of motion, manual muscle tests, gait and functional analyses, body part measurements) to aid in establishing or revising specific treatment programs.
  • Review and evaluate physician or podiatrist referral and patient medical record to determine if physical therapy is required.
  • Demonstrate comprehension and adherence to regulatory requirements and home care policies regarding practice, documentation content and timelines; complete functional assessments and documentation per regulations.
