Job SummaryWe are seeking a compassionate and experienced Nurse Practitioner (NP) to join our mobile wound care team. The ideal candidate is a self‑motivated clinician who enjoys working independently and managing chronic and acute wounds and building relationships with patients and partner facilities.
Responsibilities- Provide on-site wound assessments, debridement's and treatment for patients across various care settings.
- Collaborate with physicians, facility staff and caregivers to develop and implement care plans.
- Document all visits and treatments accurately in the EMR system.
- Monitor patient progress and adjust treatment plans as necessary.
- Educate patients and their families on wound care management and prevention strategies.
- Collaborate with other healthcare providers to ensure continuity of care.
- Maintain compliance with all state and federal regulations, including HIPPA.
- Stay current with advancements in wound care techniques, technologies, and protocols.
- Current Nurse Practitioner license in the state of Illinois. (Required)
- Current Nurse Practitioner license also in the state of Missouri. (Optimal)
- Certification as a WOCN, Adult-Gerontology, Family or Acute Care NP but will consider all candidates.
- Wound care experience preferred but not required - training available for the right candidate.
- In home care experience preferred but not required.
- Candidate may be required to travel to New Orleans, LA for 2-3 weeks for one-on-one training.
- Valid driver's license and reliable transportation.
- Strong communication, organizational and clinical skills.
- Comfort with EMR systems and mobile documentation tools.
- Ability to work independently and manage a flexible, mobile schedule.
