Home Health Care Licensed Practical Nurse

Position Overview

We are seeking a Licensed Practical Nurse (LPN) to provide skilled home care nursing visits across central Illinois counties including Champaign, Vermilion, Edgar, Iroquois, Ford, Coles, Douglas, Piatt, Moultrie, and Macon counties.

• Coordinating care plans and educating patients/families.
• Performing skilled nursing assessments and care, including complex wound care (wound vac, ostomy), blood draws, and monitoring health conditions.
• Providing direct nursing care under the supervision of an RN, mid-level provider, or physician.
• Documenting care accurately and completing documentation per regulatory requirements.
• Reporting patient responses to registered nurse or other provider for evaluation and plan modification.

• Active IL LPN license.
• Proof of auto insurance.
• Completion of an accredited LPN program.
• Minimum 2 years of nursing experience.
• Ability to travel reliably as required by the daily schedule.
• Compliance with all regulatory regulations and documentation requirements.
