Nurse Coordinator (Registered Nurse) – Assisted Living & Memory Care
Clark Lindsey’s Meadow Lark Assisted Living seeks a compassionate Nurse Coordinator to provide clinical leadership and coordinate resident care in our Assisted Living and Memory Care community. This role partners with residents, families, physicians, and team members to deliver person-centered care that promotes dignity, independence, and quality of life.

Resident Care & Clinical Coordination- Assess residents and develop individualized care plans.
- Monitor resident health status and communicate significant changes to physicians, families, and team members.
- Update care plans as needed.
- Promote person-centered care that honors resident preferences and independence.
- Provide oversight, direction, and support to Meadow Lark nurses, CNAs and resident assistants.
- Assist with team member onboarding and ongoing development.
- Uphold Meadow Lark’s standards of accountability, professionalism, teamwork, and positive resident outcomes.
- Administer and document medications according to nursing standards and state regulations.
- Maintain medication security, organization, and compliance.
- Support infection prevention practices, vaccinations, screenings, and required health tracking.
- Build positive relationships with residents, families, and team members.
- Participate in quality improvement, meetings, and educational programs.
- Support residents in maintaining their highest level of functioning and well-being.
- Graduate of an accredited nursing program with a current Illinois RN license.
- Experienced geriatric nurse with a strong understanding of dementia-related conditions and effective care strategies.
- Experience with Point Click Care, MS Office including Word, Excel, Teams and SharePoint.
- Exceptional assessment, communication, critical thinking, leadership and organizational skills.
- Ability to lift up to 50 pounds and assist with resident transfers.
- Ability to stand, walk, bend, and respond quickly in emergencies.
- Ability to work in an environment with potential exposure to blood and bodily fluids.
This full-time, salaried position will have a Monday–Friday schedule. Participation in a on-call rotation is required.
Benefits and CompensationPay rate: $80,000 - $90,000, paid bi-weekly. Company-wide bonuses equaling up to 2% of salary are available based on performance, generally in the spring and fall each year.
