LPN - Convenient Care Plus

Overview

The Licensed Practical Nurse (LPN) provides direct nursing care to assigned patients under the supervision of a registered nurse (RN) or other provider in patient care departments. Nursing services are delivered in accordance with the LPN scope of practice as defined by the Illinois Board of Nursing.

• Provide direct nursing care to patients under the supervision of a registered nurse, midlevel provider or physician.
• Perform nursing care guided by provider orders and the nursing plan of care within the LPN scope of practice.
• Report patient responses to care to the registered nurse or other provider for evaluation, intervention, and plan modification.
• Ensure documentation of care is complete and accurate.
• Administer medications orally, rectally, intramuscularly, and by inhalation.
• Assist providers with procedures such as vaginal exams, suture and staple placement, and incision and drainage.
• Act as a member of the Code Team when needed.
• Serve as a preceptor for new staff.
• Clean instruments.
• Room patients by collecting pertinent information, including chief complaint, vital signs, and medication reconciliation upon arrival.
• Conduct telephone interactions—communicate with patients, families, pharmacies, and other health care facilities as directed.
• Maintain patient information via telephone according to departmental processes and policies.
• Communicate test results to patients via phone and the My Carle nurse inbox.
• Use approved Telephone Triage books under the oversight of a registered nurse or provider.
• Perform office procedures as directed and per departmental competencies.
• May perform procedures such as suture or staple removal, application or removal of one‑step splints, crutch training, catheterizations, IV hydration therapy, and reading TB skin test results.
• Provide patients with written instructions regarding procedures, medications, or follow‑up as directed by the provider.
• Assist with prescription medication refills per department protocols.
• Call medications into pharmacies, notify patients of refills, and document actions within the EMR per provider orders.
• Maintain departmental competencies related to point‑of‑care testing, equipment handling, and specialty training.
• Perform waiting room and exam room rounding.
• Collect and process specimens correctly.

• Licensed Practical Nurse (LPN) licensed by the Illinois Department of Financial and Professional Regulation (IDFPR).
• Basic Life Support (BLS) certification within 30 days of employment.
• College diploma in Licensed Practical Nursing (required).
• Work experience in a clinical nursing setting (specific experience not stated).

The compensation range for this position is $24.10 per hour – $39.23 per hour. This represents a good faith minimum and maximum range for the role at the time of posting. The actual compensation offered to a candidate will be dependent on a variety of factors including, but not limited to, the candidate’s experience, qualifications, location, training, licenses, shifts worked and compensation model.
