Bridge to Home Housing & Employment Case Manager

Position Overview: The Cunningham Township Supervisor's Office (CTSO) employs a Bridge to Home Housing &Employment Case Manager to support residents experiencing homelessness to access housing and employment support towards independence. This position provides trauma-informed care and skilled support for individuals and families to increase health and wellness, deepen life skills, connect to community, achieve education and income goals, and grow resiliency and self-reliance.

Cunningham Township is a public office led by elected Supervisor Danielle Chynoweth that provides assistance for residents facing housing insecurity and homelessness, unemployment, disability, andfood insecurity. Our diverse team is dedicated to advocating for and with our neighbors in need, guidedby the principles of social justice, equity, cooperation, and love.

Essential Duties:

• Screen individuals referred to the Bridge to Home Program to assess eligibility.
• Intake and assess participant needs, set goals, create individualized service plans forhousing and employment, and set exit strategies.
• Conduct weekly meetings with participants assessing progress, setting goals, and making interventions.
• Support residents as needed to address emergent care needs.
• Support participants with income, budgeting, credit repair, rental history, and supportive services as needed.
• Attend medical appointments, housing-related appointments, and court-hearings with participants as needed.
• Coordinate referrals to other community resources to meet additional needs.
• Support successful exits.
• Build partnerships with outside agencies providing referrals and support.

Other:

• Maintain complete, accurate, and confidential participant files.
• Maintain up-to-date and thorough documentation.
• Track and report on program outcomes.
• Make recommendations for program improvements.
• Participate in weekly team meetings.
• Participate in staff training and professional development opportunities.
• Other duties as assigned.

Qualifications Include:

• At minimum, a Bachelor’s degree or 4 years of experience in a human services field.
• Ability to support individuals facing homelessness, joblessness, substance abuse, or mentaland physical health issues in a non-judgmental way.
• Deliver services through a trauma-informed framework that emphasizes client strength sand resilience.
• Ability to effectively organize, complete, and evaluate multiple assignments.
• Excellent oral and written communication skills.
• Strong work ethic, good judgment, and a results-oriented approach.
• Ability to follow policy and procedures, and hold participant information in strict confidence.
• Ability to adapt to changing environments and new technologies.
• Digital office skills including:
  Windows, Word, Excel, Outlook, Google docs.
• Demonstrated ability to work effectively and respectfully with people of all economic, racialand gender identities.
• Must have a valid driver's license.

Preferred but not required:

• Social Work licensure in good standing.
• Spanish, French, or American Sign Language proficiency.
