The Case Manager I – Housing Stabilization makes a direct impact on people’s lives in Champaign County by supporting individuals and families experiencing or at-risk of homelessness to find and maintain housing. This role is an integral part of the collaborative Community Services Division Housing and Homeless Services team.
The case manager will become an expert in their assigned programs, and help individuals to:
- address needs contributing to current or past homelessness,
- obtain required documentation and supports for housing,
- apply for housing opportunities,
- problem solve housing challenges,
- connect with other community providers,
- increase income and strengthen budgeting success,
- build advocacy and life skills, as well as
- achieve individualized goals focused on maintaining or increasing housing stability.
Ideal candidates will:
- Have entry- to mid-level knowledge and experience in human social services, particularly with people impacted by homelessness, mental health, or substance use disorder,
- Be organized, collaborative, trauma-informed, and strong communicators and problem solvers, and
- Show initiative and interest in learning and growing their capacity to support housing stabilization.

- Associate degree in human services or related field; combination of education, training, and experience in the field may be considered. One year of experience in human services and/or bachelor’s degree preferred.
- Ability to pass criminal background check.
- Valid driver’s license, safe and reliable vehicle, and the ability to transport clients and their children and visit their homes throughout service area daily using an agency-provided vehicle.
- Strong background in customer services and human services with program’s target population.
- Possess household budgeting skills.
- Understands steps to implement to work toward gainful employment for individuals lacking employment history.
- Knowledge of public assistance programs, including eligibility and application process.
- Ability to attend community meetings, provide program information at community meetings, and collect and share relevant meeting information with team.
- Ability to attend worksite regularly to perform job duties and responsibilities.
- Ability to use motivational interviewing to assist clients in setting personal goals toward stability.
- Computer skills needed to complete case documentation, data entry and timesheets.
- Ability to work a flexible schedule to accommodate client scheduling needs including periodic evenings and weekends.
- Preferred: Knowledge of and or experience working with persons impacted by homelessness, mental health and or substance abuse preferred.
- Preferred: Knowledge of trauma-informed care principles.
- Preferred: Knowledge of and or experience in working with individuals with disabilities and/or medically fragile individuals.
- Excellent customer service skills with a professional demeanor, networkingskills, and the ability to exercise good judgment in complex situations.
Schedule:
Hours primarily between 8:00 a.m. – 4:30 p.m., M-F (Flexibility is necessary to support the goals of the program.)
