The Champaign County Board of Developmental Disabilities (CCBDD) is seeking a Service and Support Administrator to join its team in providing essential services to individuals with developmental disabilities. This position plays a vital role in ensuring individuals receive the necessary support to meet their unique needs while promoting independence and community inclusion.
Job Responsibilities
The Service and Support Administrator will be responsible for assessing individual service needs and developing Individual Service Plans (ISPs) through a person-centered approach. Key duties include:
- Coordinating and facilitating ISP conferences, ensuring seamless service planning to reduce duplication.
- Advocating for individuals and their families, ensuring they are aware of their rights and available services.
- Facilitating team meetings and coordinating communication between enrollees, families, and service providers.
- Identifying and accessing community resources to support individuals' goals and needs.
- Monitoring and evaluating service implementation to ensure quality and effectiveness.
- Ensuring compliance with health and safety regulations, including reporting Major Unusual Incidents (MUIs).
- Maintaining accurate records and documentation for Medicaid billing and regulatory purposes.
