LPN Field Associate

CRSI seeks an LPN Field Associate to deliver high-quality nursing care in community and home-based settings within our Healthcare & Medical Services department. In this field-based role, you will perform patient assessments, administer medications and treatments within LPN scope, and document care using EHR systems. You'll collaborate closely with RNs, physicians, and interdisciplinary teams to support individualized care plans grounded in safety and industry best practices.

Responsibilities

• Provide in-home nursing care to patients as an LPN Field Associate, including monitoring vital signs and administering prescribed treatments within scope of practice.
• Document patient status and interventions accurately in EHR and communicate changes promptly to RNs, physicians, and interdisciplinary team members.
• Support development and implementation of care plans focused on safety, quality, and best practices in home and community-based settings.
• Educate patients and families on medications, self-care techniques, and disease management to promote independence and prevent readmissions.
• Adhere to infection control, safety protocols, and regulatory standards while representing CRSI's culture of collaboration and innovation in the field.
• Participate in ongoing training, quality improvement initiatives, and case conferences to improve outcomes and refine standards of care.
