Shift: Friday - Sunday | 7:00 AM - 7:00 PM (rotating on-call)

As a Home Health RN with UnityPoint at Home, you’ll provide one-on-one care to patients with total autonomy over your role. You will manage patient admission, care planning, and discharge, ensuring high-quality care in patients’ homes.

• Make visits to patients' homes and conduct interdisciplinary assessment.
• Perform initial and comprehensive assessments in accordance with agency policies.
• Perform nursing assessment at each visit.
• Ensure quality and safe delivery of services according to the care plan.
• Use equipment and supplies safely, effectively, and efficiently.

• Graduate of State Board approved program for Registered Nurses.

• Valid license as Registered Nurse in the state where providing care.
• Valid licensed driver with automobile insurance per state/or organization requirements.
• Mandatory Reporter:
  Child & Dependent Adult Abuse.
• CPR:
  Maintain a valid Basic Life Support (BLS) Healthcare Provider Card with re-certification.

• One year nursing experience caring for similar patient population as to be assigned.
