Current Registered Nurse (RN) license in the state or compact state RN license.
Certifications:
Current BLS Certification.
Must-Have:
Valid driver’s license and reliable transportation required for home visits.
Description:
We are seeking a dedicated and compassionate Hospice Registered Nurse (RN) to join our interdisciplinary team.
The RN will provide skilled nursing care and support to terminally ill patients and their families in accordance with the patient's plan of care and hospice philosophy.
Key responsibilities include performing comprehensive assessments of patients' needs and coordinating individualized care plans.
The RN will provide pain and symptom management consistent with the hospice plan of care.
You will educate and support patients and families regarding the disease process, self-care techniques, end-of-life issues, and the hospice philosophy.
Collaboration with the interdisciplinary team including physicians, social workers, chaplains, and volunteers is essential.
The RN will participate in an on-call rotation.
Maintaining accurate and timely documentation in compliance with state, federal, and organizational policies is required.
Strong clinical assessment skills and excellent communication, interpersonal, and organizational skills are critical.
Compassionate demeanor and ability to support patients and families during difficult times is vital.
On-call shifts may require responding within 45 minutes to patient needs.
Ability to work holiday shifts is expected.
Experience with EMR systems is preferred.
