Hospice RN
Location: Moab, UT
Contract Type: Travel Contract
Shift Type: Days (exact start/end times not specified);
Day-shift role with an on-call rotation of 1-3 shifts per week (able to respond within 45 minutes when on call);
Includes approx. 3 weekends during the 13-week contract and holiday coverage as needed
Hours per Week: 32
Candidate Pay Rate: $63 per hour

- Perform comprehensive clinical assessments and create individualized care plans
- Provide expert pain and symptom management consistent with each patient’s hospice plan of care
- Educate and support patients and families about the disease process, self-care, and end-of-life planning
- Coordinate care and collaborate closely with physicians, social workers, chaplains and volunteers
- Deliver skilled nursing care during home visits and respond promptly when on-call
- Maintain accurate, timely documentation in compliance with state, federal and organizational policies
- Current Registered Nurse (RN) license in Utah or compact state RN license
- Minimum 3 years of nursing experience (hospice, home health or palliative care preferred)
- Current BLS certification
- Valid driver’s license and reliable transportation for home visits
- Ability to respond within 45 minutes while on-call
- Willingness to participate in weekend and holiday coverage as scheduled
- Active Registered Nurse (RN) license (UT or compact)
- Current BLS certification
- Strong clinical assessment and critical thinking skills
- Proficient in pain and symptom management for terminally ill patients
- Excellent documentation and compliance practices
- Skilled at patient and family education with a compassionate approach
- Experience with Hospice Soft and/or Careficient EMRs is a plus
- Effective interdisciplinary collaboration and communication skills
- Organizational skills for caseload management and timely charting
- Travel-friendly contract with support for clinicians new to the area
- Mileage reimbursement for home visits (amount provided by employer)
