Job Summary:
The registered nurse plans, organizes, and directs home care services, focusing on community health education and experience. The professional nurse builds from community resources to plan and direct services to meet the needs of individuals and families within their homes and communities.
- Complete initial and ongoing assessments of patients and families.
- Develop and revise care plans based on nursing diagnosis and incorporate therapeutic, preventive, and rehabilitative actions.
- Administer medications and treatments as prescribed by physicians.
- Provide direct patient care in accordance with the State Nurse Practice Act.
- Counsel patients and families to meet nursing and related needs.
- Provide health‑care instructions based on assessment and plan of care.
- Implement discharge planning and coordinate care transitions.
- Act as case manager when assigned to coordinate care for an assigned caseload.
- Prepare clinical notes and update primary physicians at least every 60 days.
- Communicate with physicians regarding patient needs and report any changes in condition; obtain or receive physician orders as required.
- Coordinate care with community health professionals.
- Participate in on‑call duties as defined by the on‑call policy.
- Ensure arrangements for equipment and other necessary items and services are available.
- Supervise, instruct, and evaluate home health aide care every two weeks.
- Graduate of an accredited school of nursing.
- 1–2 years of recent acute care experience in an institutional setting.
- Current state licensure and CPR certification.
- Bachelor’s degree with at least 1 year of home‑health care experience preferred.
- Basic math, observation, verbal and written communication, problem‑solving skills.
- Ability to walk, stand, lift, position, transfer patients, and lift supplies and equipment; ability to reach, stoop, bend, kneel, and crouch.
- Visual acuity and hearing adequate to perform required nursing skills.
- Licensed driver with an automobile in good working order and insured in accordance with organizational requirements.
