• Conduct thorough assessments and develop personalized, comprehensive care plans.
• Guide and educate patients and families on health maintenance and disease prevention.
• Assist with Activities of Daily Living (ADLs) with dignity and respect.
• Coordinate schedules directly with patients and align with their plan of care, approved hours, and preferences.
• Monitor patient symptoms and promptly report changes to the care team.
• Administer medications and treatments as prescribed.
• Communicate seamlessly with Case Managers, physicians, and specialists.
• Maintain accurate and heartfelt patient logs in the EMR system.
• Provide emotional support and become a trusted presence in patients’ lives.

• Prepare clear, compassionate clinical notes that tell the patient’s story.

• Valid and active RN license in the state of practice.
• Willingness to travel and provide care at patients’ homes.
• Graduation from an accredited nursing school.
• Current BLS certification.
• Ability to pass state and federal background checks.
• Must hold malpractice insurance.
• Strong problem‑solving and critical‑thinking skills with a warm, empathetic approach.

USD $40.00 – $45.00 /hr.
