Job Summary
The Licensed Practical Nurse (LPN) provides general nursing care to patients, families and caregivers in the Medical Group practices. The LPN assists and advocates for patients and families health in support of the patient-centered approach and delivery model of care, and is an active member of the care team. The LPN collaborates with physicians/producers, registered professional nurses, and multidisciplinary care team members.
The LPN performs tasks within the framework of case finding, health coaching, and supportive care under the direction of a registered professional nurse or licensed physician or licensed health care provider. Nursing is performed in accordance with the standards outlined for LPN under the NYS Education Law, NYS Office of the Professions, and the State Nurse Practice Act. The LPN follows the mission, vision, I-CARE values, policies, and procedures of Mohawk Valley Health System.
- Gather patient measurements and collect patient data
- Record and report patient findings
- Prepare patient for examination and treatment
- Assist with examination and treatments
- Perform tasks within the framework of supportive care under the clinical direction of a licensed health care provider or registered professional nurse
- Support self-care, self-management, and the established plan of care
- Utilize standard precautions and practice at all times
- Maintain equipment, supplies, inventory, and a safe and clean work environment
- Maintain technical knowledge by attending educational workshops, mandatory in-services; reviewing publications and participating in department committee meetings or work groups
- Engage in quality improvement initiatives, including continuing education and participation in system-wide and department-specific teams
- Maintain a cooperative relationship among the healthcare team by effectively communicating information
- Respond to patient requests and communicate effectively to the licensed health care provider who can fulfill the patient request
- Graduate of an accredited school of licensed practical nursing program
- Preferred:
Bachelor's or Master's of Science in Nursing from an accredited nursing program
- Current New York State licensure as a Licensed Practical Nurse;
Basic Life Support Certification – must receive BLS certification within six weeks of hire date
