Duration: 13 Weeks (Extendable)
Shift: Evening Shift - 2:30 PM to 10:30 PM
Schedule Requirement: Every other weekend required
Weekly Gross Pay: $1,143 - $1,500
Position Overview

A Long-term care facility is seeking a dependable Licensed Practical Nurse (LPN) to provide high-quality nursing care to residents in a skilled nursing and rehabilitation setting. The ideal candidate is flexible, team-oriented, and comfortable floating within the facility.
Facility & Unit Details

• Setting: Long-Term Care / Skilled Nursing
• Unit Size: 40-bed unit
• Floating: Required to all units within the same building
• Guaranteed Hours: 37.5 hours per week
• Patient Populations:
  • Medical-Surgical
  • Orthopedic
  • Dementia
  • Memory Care
• EMR System: Point Click Care

Responsibilities

• Administer medications and treatments per physician orders
• Monitor, document, and report changes in resident condition
• Perform wound care and basic nursing procedures within LPN scope
• Assist with activities of daily living (ADLs) as needed
• Collaborate with RNs, CNAs, and interdisciplinary team members
• Maintain accurate documentation in the electronic medical record
• Ensure resident safety, dignity, and quality of care

Required Qualifications

• Active New York State LPN License
• BLS Certification (AHA preferred)
• Minimum 1+ year of Long-Term Care or skilled nursing experience
• Ability to float across units
• Strong communication and documentation skills

• 75-mile radius rule applies for travel candidates
• Local candidates are welcome and eligible for local rates
• Every other weekend availability is mandatory
