LPN – Long Term Care
Schedule:
Shift: Evenings 2:30 PM - 10:30 PM Every other weekend required
Assignment Details:
- Contract Length: 13 weeks
- Guaranteed
Hours:
37.5 hours per week
Requirements:
- 1+ years of experience
- BLS certification
- New York license
- Familiarity with EMR systems, specifically Point Click Care
Roles & Responsibilities:
- Provide comprehensive care to residents within a long-term care setting.
- Administer medications and treatments as prescribed.
- Monitor patient health and document observations.
- Communicate effectively with patients, families, and healthcare team members.
- Assist with activities of daily living and support the emotional and physical well-being of residents.
- Collaborate with multidisciplinary teams to create care plans.
