Key Responsibilities of the LPN/LVN – Rehabilitation – Travel

• Provide nursing care to residents in an LTC facility
• Administer medication and treatments as prescribed
• Document and report any changes in patient condition
• Collaborate with healthcare team to ensure high-quality care
• Float within the building as required by staffing needs

Requirements of the LPN/LVN – Rehabilitation – Travel

• 1+ years of experience in a long-term care facility required
• Current BLS certification
• Active LPN NY License and all other active licenses verified by NURSYS
• Experience with EMR systems, specifically Point Click Care
• Valid driver's license with matching address to demographics
