Zideris zoekt een zorgconsulent die als eerste aanspreekpunt fungeert voor mensen met een verstandelijke beperking en hun netwerk. Je luistert, adviseert over mogelijkheden binnen onze organisatie en begeleidt hen tijdens het aanmeld- en plaatsingstraject.
Je werkt aan werving en inschrijving van cliënten en onderhoudt relaties met zorgaanbieders. Je draagt bij aan tijdige indicaties en aanvragen, schakelt tussen casussen en werkt zowel zelfstandig als in een team.
