Tutor

This person provides direct instruction to students outside of the school day. Instruction may be provided in a school building, or public building, or in the student's home.

* Confers with classroom teacher (s), counselor, social worker or other school personnel on the student's needs.

* Develops and completes instructional lessons.

* Completes assessments and reports as may be needed.

* Provides the student and the family with supplementary instructional material as may be needed to reinforce lessons.

My application affirms, that I have read and understand that the information which I have provided with this application process and the accompanying resume is true and complete to the best of my knowledge. I agree that any omission or falsified information shall subject me to disqualification from further consideration for employment and shall be considered justification for my immediate dismissal if discovered at a later date.

I hereby authorize all persons, schools, current employer(s) and previous employers and other organizations named in the application process and accompanying resume to provide Uxbridge Public Schools with any relevant information that may be helpful in arriving at an employment decision.
