We have a vacancy in Hillingdon for a full-time Band 5 community staff nurse in the School Nursing Service.

You will hold a first level registration adult, mental health or child branch and if you are due to qualify shortly you are also welcome to apply.
Main duties of the jobYou will have the ability to work autonomously as well as part of a skilfully mixed team. You will also be flexible, responsive and keen to modernise practice and influence health and Trust policies. You will have enhanced interpersonal skills to identify low protective factors and indicators of abuse and neglect, or anxiety and low confidence in young people at the earliest opportunity and through your skills, you will develop strength‑based plans of care to build resilience, promote positive wellbeing and improved health outcomes for young people.

Clinical Responsibilities
- Deliver the Healthy Child Programme.
- Undertake holistic health needs assessments of children across a wide range of environments and a broad population spectrum.
- Interpret, analyse and collate complex information, liaising with children, young people and families, local School Nurses and other services and agencies as appropriate.
- Implement a plan of care using evidence‑based criteria, rationalise professional decisions and actions, monitor progress and adapt plans to maximise health benefits for clients.
- Provide skilled, evidence‑based nursing care which adheres to agreed policies and procedures.
- Provide ongoing support for individuals and families, sometimes delegated by senior staff, reporting back to senior colleagues.
- Signpost and refer to other specialist services for support and intervention.
- Help children, parents and carers identify their own level of knowledge and skill and offer support, information and advice to enable them to access services and adopt healthy lifestyles.
- Use excellent communication skills to communicate effectively with children, parents/carers and other professionals.
- Maintain accurate and comprehensive record keeping in line with NMC guidance and Trust policy.
- Have knowledge of Information Sharing and Assessment protocols and the ability to identify and discern relevance of information to be shared to promote the safety of children.
