Job OverviewWe are hiring a Housing Care Manager to support residents placed in Short-Term Post-Hospitalization and Recuperative Care housing. This role combines resident engagement, care coordination, housing navigation, and ownership of required documentation in our electronic health record (EHR) software.
You will work directly with residents, understand where they are in their recovery and housing journey, coordinate next steps, and ensure all housing services are documented accurately, timely, and in alignment with program requirements.
This role is ideal for someone who is detail-oriented, organized, and comfortable building relationships with people during critical transition periods.
Who We ServeResidents in Short-Term Post-Hospitalization and Recuperative Care housing are individuals who are medically stable but not yet ready to return to independent living. Many are recovering from hospitalization, illness, injury, or acute health events and need a safe, supportive environment to stabilize while planning their next housing step.
Residents may have complex social needs, limited support systems, and housing instability. This role helps ensure their housing stay is purposeful, coordinated, and clearly documented.
Key ResponsibilitiesResident Engagement and Care Coordination
- Conduct regular check-ins with residents to understand needs, goals, and progress
- Support residents in stabilizing after hospitalization or acute health events
- Coordinate housing-related care activities during the resident's stay
- Assist with discharge planning and transitions to the next level of housing
- Communicate effectively with house staff and external partners
- Own daily documentation for all Short-Term Post-Hospitalization and Recuperative Care residents
- Complete required housing notes that reflect daily engagement, support, and progress
- Ensure documentation aligns with authorized housing services and length of stay
- Maintain accurate, timely, and organized resident charts
- Prevent documentation backlogs through daily follow-through
- Deliver Housing Navigation activities when authorized
- Support residents with housing searches, applications, and placement steps
- Coordinate with housing providers and community resources
- Document Housing Navigation activities clearly and consistently
- Track housing progress and outcomes
- Ensure housing services are delivered as authorized and documented accordingly
- Monitor resident length of stay and support timely, appropriate discharges
- Respond promptly to documentation feedback or quality reviews
- Use established templates and workflows consistently
Short-Term Post-Hospitalization and Recuperative Care
- Daily housing notes completed on time for 100 percent of active residents
- Documentation reflects ongoing engagement and support throughout the stay
- Resident length of stay tracked and aligned with authorization and program limits
- Discharge documentation completed promptly and accurately
- Zero billing delays due to missing or incomplete housing documentation
- Housing Navigation activities delivered only when authorized
- Monthly Housing Navigation documentation completed accurately and on time
- Clear tracking of housing search, applications, and placement efforts
- Progress toward housing placement documented consistently
- Zero rolling documentation backlog
- Resident charts maintained in audit-ready condition at all times
- High accuracy and consistency across all housing documentation
- Timely correction of documentation issues when identified
- Residents feel supported, heard, and guided during their housing stay
- Housing services are clearly documented and easy to understand
- Charts are always current and ready for review
- Discharges and transitions are coordinated smoothly
- Housing navigation efforts are visible and well-tracked
- Strong attention to detail and organization
- Comfortable building rapport and speaking with residents regularly
- Ability to manage daily responsibilities without falling behind
- Ability to coordinate and communicate with team members with the goal of providing coordinated care
- Clear written communication skills
- Experience in housing, care coordination, case management, social services, or similar fields preferred
- Previous experience with documentation systems or EHRs is a plus
Accurate documentation and strong care coordination protect program integrity and ensure residents receive meaningful support during a critical transition period. This role plays a direct part in resident outcomes, program success, and sustainability.
