The (LPT) or (LVN) supports the health and well-being of individuals by administering medications, performing nursing procedures, monitoring health conditions, and advocating for individuals in alignment with their Individual Program Plans (IPP).

• Administer medications following the Five Rights of Medication Administration
• Perform nursing procedures including injections (IM, insulin), g-tube feedings, colostomy care, wound care, and breathing treatments
• Monitor and assess individuals for changes in behavior, health status, or signs of infection
• Take and document vital signs and conduct basic head-to-toe assessments
• Attend medical appointments and communicate changes or concerns with physicians
• Support individuals with activities of daily living (ADLs) as needed
• Respond appropriately to behavioral episodes using CPI techniques
• Complete daily documentation and report incidents or concerns
• Participate in community outings and activities with individuals
• Perform other related duties as assigned

• Active California (LVN) or (LPT) (required)
• Minimum 1 year experience supporting adults with developmental disabilities
• CPR/First Aid and CPI certification (required)
• Must be 21 years of age or older
• Valid California Driver’s License
• Ability to pass all pre-employment requirements, including a background check
