Job Description
At North Bay Health, the Primary Care Team Registered Nurse (Team RN) functions as a member of the multidisciplinary team, utilizing clinical knowledge and competence, positive communication skills, problem‑solving and conflict‑resolution techniques, and strong assessment, organization and time‑management skills. The Team RN is familiar with the case‑management process, focusing on customer‑service skills, goal setting, and outcome measurement to ensure optimal client outcomes while considering payer requirements.
The Team RN collaborates with healthcare teams across the North Bay Health Care System—at the appropriate level—in collaboration with patients, families, physicians, staff and administration, and works under the direction of the Manager and Director of the Centers for Primary Care.
Education
- Graduated from an accredited nursing school or college.
- BSN required.
Licensure/Certification
- Current California state RN license required.
- AHA or equivalent BLS required.
Experience
- One year inpatient or equivalent nursing experience required.
- Relevant outpatient clinical experience preferred.
- Minimum of one year case‑management experience preferred; equivalent case‑management education considered.
- Current knowledge and skill in providing ambulatory nursing care for Geriatric, Adult, Young Adult, Pediatric and Neonatal patients.
- Computing skills, including experience with MS Office (Excel, Word, PowerPoint, etc.) and data management.
- Effective communication with all levels of patients, physicians, health‑care personnel, supervisory staff and peers.
- Exceptional organizational and prioritization skills with minimal oversight.

Compensation note: the listed range includes a per diem differential.
