Job SummaryWe are seeking a compassionate, dependable, and clinically skilled Per Diem Home Health Registered Nurse to join our growing home health team.
The Home Health RN provides professional skilled nursing services within the scope of nursing practice standards and under the supervision of the Director of Patient Care Services. The RN works closely with physicians, patients, families, and the interdisciplinary care team to provide high-quality, patient-centered care in the home.
This role is ideal for an RN who enjoys autonomy, meaningful one-on-one patient care, strong team communication, and the opportunity to help shape a growing home health division with a respected local agency.

- Provide skilled nursing services requiring substantial and specialized nursing knowledge.
- Complete comprehensive patient assessments, including medical history, physical assessment, home safety review, and environmental assessment.
- Perform assessment visits and complete timely documentation, including OASIS assessments and skilled nursing notes as required.
- Develop, implement, and revise individualized Plans of Care in collaboration with the physician and interdisciplinary team.
- Identify patient needs and determine when additional home health services may be medically necessary.
- Regularly re-evaluate the patient’s nursing needs, functional status, ADL and iADL abilities, and need for home health aide services.
- Develop, revise, and sign the Home Health Aide plan of care when HHA services are ordered.
- Supervise Home Health Aide services in accordance with state and federal requirements and document supervision timely.
- Provide patient and family education regarding disease management, medications, self-care techniques, safety, and the plan of care.
- Communicate patient status changes, needs, and concerns to the physician, Director of Patient Care Services, Nursing Supervisor, and other disciplines as appropriate.
- Coordinate patient services with nursing, therapy, social work, home health aide services, physicians, payers, and community resources.
- Participate in case conferencing to support best practices and quality patient outcomes.
- Submit timely requests for re-authorization or additional visits when more visits are needed.
- Review billing-related concerns with the patient and/or family when applicable, including situations where co-pay or Medicare coverage may not apply.
- Participate in clinical record reviews, performance improvement activities, staff meetings, and in-services as needed.
