Position Summary: Home Care is in high demand and we are looking for experienced and caring LPNs to join our team at Home Health Care Incorporated!
Successful applicants will have a proven track record as an LPN, with home care experience a huge benefit but not required. We encourage you to apply today!
Responsibilities:
- Provide patient care according to client’s care plan and needs
- Perform woundcare treatments and assist clients with medication management
- Teach theclient and/or family members about care and support skills
- Communicate regularly with RN Case Managers, Supervisors, and the Care Team
Qualifications:
- Licensed as aPractical Nurse / LPN in the state of Minnesota
- Driving is required; must have a reliable vehicle with current insurance and a valid
Driver's License - Demonstratedability to work autonomously
- Prior Nursing experience in a TCU, Hospice, or Home Health setting
- Experience with Electronic Medical Records (EMR) software is preferred
Military Occupational Classification (MOC) Codes: If you're amilitary member transitioning to civilian life, we value your service andskills. Applicable MOS codes for this role include:
- Air Force: Practical Nursing Specialist (4N0X1)
- Marines: Hospital Corpsman (HM)
