The Community Support Services Southeast Mobile team provides services and requires frequent travel in the following counties:
Anoka, Hennepin, Ramsey, Washington, Craver, Dakota and Scott. When a State‑owned vehicle is not available and an employee is required to use their personal automobile to conduct authorized State business, the employee will be reimbursed at the current Federal IRS mileage reimbursement rate.

The Positive Support or Behavioral Health Consultant will have the opportunity to help people live better lives, work with a variety of community providers, and work with a great Community Support Services team.

Given the integrated nature of Community Support Services work, the Positive Support Consultant demonstrates expertise in behavioral assessment and interventions with consultation as needed from colleagues with complementary expertise. As a bridge connecting providers together in the best interest of the individual, the Positive Support Consultant integrates multidisciplinary expertise to facilitate positive outcomes for individuals and their support networks. In each of these interactions, the Positive Support Consultant strongly impacts the quality of individuals’ lives and successful community living.

Primary

Responsibilities
  • Completing clinical assessments for individuals with complex behavioral health challenges to identify appropriate community supports for them and their support networks.
  • Providing support and/or crisis prevention and response plans to clients and their supports.
  • Serving as a resource, liaison and technical assistant to coordinate care amongst individuals served, their families, providers and other stakeholders.
  • Identifying training needs, developing and presenting training to enhance the community’s capacity to support individuals with complex behavioral health challenges in the least restrictive community settings.
  • Safeguarding clients’ human rights through the use of Positive Behavior Support methods which are the least restrictive and intrusive practices appropriate for the situation.
  • Participating as a Community Support Services team both locally and statewide.
Minimum Qualifications

Three (3) years of experience providing behavioral health clinical services including at least 4 of the 6 activities listed below:

  • Delivering psychoeducation
  • Providing case coordination among multidisciplinary team members
  • Conducting comprehensive assessments
  • Leading development of treatment/support plans as required by program licensure
  • Planning discharge/transition and/or aftercare
  • Developing behavioral interventions and recommendations for interdisciplinary staff implementation
