SCHEDULE
: PRN, 8 HR Day Shift, 8am-4:30pm M-F
Position Summary
. Responsible as a member of the interdisciplinary team for assigned functions related to patient admissions. Collaborates with the Hospice Medical Director to help ensure eligibility and appropriateness for admissions to the hospice program. Performs initial patient assessments and assists in the development and implementation of the initial Plan of Care. Ensures continuity of care throughout the admissions process. Provides education on hospice care and services as needed.
Supports other aspects of Hospice nursing services as census/patient need requires. Rotates weekend on-call rotation upon request.
- Required current licensure as a professional Registered Nurse in the state of Georgia, with BSN or higher professional nursing degree preferred.
- Required minimum of two (2) years varied work experience as a Registered Nurse (prior hospice/palliative care experience preferred).
- Demonstrated understanding of and adherence to the hospice philosophy and ability to work as a member of an interdisciplinary team in meeting the needs of the terminally ill.
- Certification in hospice/palliative care strongly preferred.
- Must demonstrate attention to detail, knowledge of diagnosis codes; ICD-10 experience preferred.
- Strong clinical skills in assessment, planning, intervention, and evaluation of patient care.
- Positive attitude with demonstrated ability to handle delicate situations and to communicate effectively with hospice team members, physicians, other referral sources, patients/families, and community agencies.
- Ability to work non-judgmentally with persons of diverse backgrounds and lifestyles.
- Self-motivated with strong organizational skills and demonstrated ability to use critical thinking and problem-solving techniques with minimal direction and appropriate collaboration.
- Ability to operate clinical and office equipment, including computers and telecommunications devices.
- Must have current GA driver’s license and satisfactory driving record.
- Must have current CPR certification.
