The LPN is responsible for providing quality patient care and performing skilled care in the patient’s home.

• Provide services within the scope of practice as defined by state laws and in accordance with the plan of care.
• Collaborate with physicians and RNs, documenting care accurately and promptly, and may not alter the plan of care.
• Instruct the patient’s family in the care of the patient and maintain a healthy environment.
• Observe, record, and report to the supervising nurse and/or physician the patient's response to treatment and changes in condition daily.

• One year of clinical LPN experience.
• Current LPN license in the state where services are rendered.
• Current CPR certification.
• Driver’s license, vehicle insurance, and access to a dependable vehicle or public transportation.
• Reliable commute to multiple patient residences daily within the assigned geographic area.
