JOB LOCATION : Hospice

DEPARTMENT: AR - HOSPICE

SCHEDULE: Full Time, 7on/7off,

POSITION SUMMARY

Responsible as a member of the interdisciplinary team to the Nurse Manager and the attending physician for the provision of skilled nursing assessment, planning and care in order to manage the comfort and health of patients and families consistent with Hospice of South Georgia Policies and Procedure. Responsible for providing 24hr skilled nursing intervention and supportive care in patient's home for weekend, holiday, evening and night on-call in a rotating 7on/7off schedule.

Utilizing physicians' orders, critical thinking and professional nursing skills, develops and implements a hospice plan of care that meets each patient/family's specific needs. Formulates this plan in accordance with state and federal guidelines and Hospice of South Georgia Policies and Procedure. Promotes and coordinates team work with physicians and associates; interfaces effectively with other departments and IDT members.

KNOWLEDGE, SKILLS & ABILITIES
  • Education, Certification, & Licensure:
    • Current RN License in State of Georgia.
    • Must have and maintain a valid driver's license and good driving history record in accordance with SGMC Health's insurance carrier's standards.
    • Current CPR required
    • Applicable specialty certification preferred
    • Completion of unit/service specific training program.
  • One year experience in home health or hospice setting preferred.
  • Must have positive attitude with demonstrated ability to handle delicate situations and to communicate effectively with members of the hospice team, patients/families, and community agencies/institutions.
  • Must be self-motivated and autonomous with strong organizational skills and demonstrated ability to prioritize task, handle pressure, and utilize critical thinking/problem-solving techniques.
  • Demonstrates ability to respond well to supervision.
  • Highly skilled in his/her ability to adapt to new situations.
  • Ability to read and interpret policies, standards, and think critically evaluating patient care issues.
  • Strong nursing skills in assessment, planning, intervention and evaluation.
  • Must possess ability to communicate clearly with, and care for diverse population of patients/family units of all ages, a variety of cultures, and/or persons frequently exhibiting stressed behaviors.
WORKING CONDITIONS - ADA INFORMATION
Office environment, patient homes, nursing home, personal care homes, hospital and emergency department. May frequently have high levels of physical and emotional stress in a rotating 7on/7off basis. Requires considerable standing, mental effort, stooping, bending, lifting (in excess of fifty (50) lbs.) and operation of clinical and office equipment. Likely to be exposed to blood and body fluids, including infectious diseases.

May drive long distances in all kinds of weather and be in and out of automobile. Uses medical, office equipment, and telecommunications devices on a regular basis (e.g., I.V. pumps, fax, computer, and pagers). Physical functional capacity for healthcare workers as defined by the Saunders Method (very heavy).
