Hospice
DEPARTMENTAR - HOSPICE
SCHEDULEFull Time, 7on/7off
POSITION SUMMARYAs a member of the interdisciplinary team, you will report to the Nurse Manager and attending physician, providing skilled nursing assessment, planning, and care to manage the comfort and health of patients and families in accordance with Hospice of South Georgia policies and procedures. You will deliver 24‑hour skilled nursing intervention and supportive care in patients’ homes on weekends, holidays, evenings, nights, and on‑call duties within a rotating 7on/7off schedule.
Utilizing physicians’ orders, critical thinking, and professional nursing skills, you will develop and implement a hospice plan of care tailored to each patient/family’s specific needs, in line with state and federal guidelines.
- Current RN license in the State of Georgia.
- Valid driver's license and good driving history as required by SGMC Health’s insurance carrier standards.
- Current CPR certification.
- Specialty certification preferred.
- Completion of unit/service specific training program.
- One year experience in home health or hospice setting preferred.
- Positive attitude with the ability to handle delicate situations and communicate effectively with hospice team members, patients/families, and community agencies.
- Self‑motivated, autonomous, with strong organizational skills and the ability to prioritize tasks, handle pressure, and apply critical thinking/problem‑solving techniques.
- Responsive to supervision.
- Adaptability to new situations.
- Ability to read and interpret policies, standards, and critically evaluate patient care issues.
- Strong nursing skills in assessment, planning, intervention, and evaluation.
- Effective communication with diverse patient and family populations across all ages, cultures, and behavior patterns.
The role includes office environments, patient homes, nursing homes, personal care homes, hospitals, and emergency departments. Duties may involve high levels of physical and emotional stress on a rotating 7on/7off basis, requiring standing, mental effort, stooping, bending, lifting over 50 lbs, and operating clinical and office equipment. Exposure to blood and body fluids, infectious diseases, long‑distance driving in all weather, and use of medical, office, and communication devices are expected.
Physical functional capacity is required as defined by the Saunders Method (very heavy).
