Job DescriptionWe are seeking a skilled and compassionate Nurse Practitioner (NP) to join our cardiology team. The ideal candidate will have experience in cardiovascular health and/or post‑acute and be comfortable managing patients with a variety of cardiac conditions. This role involves direct patient care, including assessment, diagnosis, treatment, and ongoing management of cardiovascular diseases in collaboration with our cardiologists.
Responsibilities- Conduct comprehensive cardiovascular assessments, including patient history and physical exams
- Diagnose and manage acute and chronic cardiovascular conditions
- Order and interpret diagnostic tests such as EKGs, echocardiograms, stress tests, and lab work
- Develop and implement treatment plans in collaboration with cardiologists
- Provide patient education on heart health, disease prevention, medications, and lifestyle modifications
- Monitor and manage patients with hypertension, heart failure, atrial fibrillation, and other cardiac conditions
- Perform follow‑up visits and manage chronic disease care
- Assist with hospital rounds and inpatient consultations as needed
- Maintain accurate and timely electronic health records (EHR)
- Collaborate with a multidisciplinary team to ensure the highest quality of patient care
- Board Certification (Nurse Practitioner)
- Valid state license and certification as a Nurse Practitioner
- Strong clinical assessment skills and the ability to interpret diagnostic tests
- Excellent communication skills, both verbal and written, with a focus on patient education
- Ability to work effectively within a team‑oriented environment
- Proficiency in electronic health record (EHR) systems
- Current ACLS Certificate
- Current BLS certificate
