Position Summary

The Maternal Fetal Nurse Navigator is a patient care professional who provides support, care coordination, and guidance for high-risk obstetric patients and their families within our health system. Collaborates with physicians, nursing and imaging staff, referring physician offices, and other healthcare team members to optimize patient care and outcomes for the Maternal Fetal Medicine patients within our health system. Provides education to patients and communicates results of diagnostic testing and procedures performed.

Coordinates follow-up appointments for patients requiring additional evaluation, testing, consultations, or procedures, including ordering and scheduling as appropriate. Performs insurance verification and patient account registration and manages all incoming and outgoing referrals for Maternal Fetal Medicine patients, including follow-up to ensure services are scheduled and results are obtained. Responsible for insurance verification, patient account registration, and obtaining authorizations for clinic visits, imaging studies, procedures, and other services related to treatment.

Education

  • Current, unencumbered license as a Registered Nurse in the state of Georgia required.
  • Bachelor of Science in Nursing preferred.
  • BLS certification required.

Experience

  • Four years nursing experience preferred.
  • Experience with patient registration/admitting, diagnosis coding, and IBM PC experience preferred.

Knowledge, Skills, and Abilities

  • Flexible in work schedule, assignments, workloads.
  • Skilled at setting priorities and utilizing problem solving techniques.
  • Ability to analyze regulations, standards, clinical issues, and patient outcomes.
  • Excellent verbal, written, and behavioral communication skills required.
  • Excellent computer skills required.
  • Ability to operate variety of office, educational, clinical and non-clinical equipment.
  • Demonstrated use of clinical skills and expertise in the area of maternal fetal health.
  • Ability to comprehensively assess patient’s physical and psychosocial status.
  • Ability to effectively and efficiently coordinate, implement and monitor the care management process and treatment plan for each patient.
  • Ability to communicate openly and empathetically with patients and families.
  • Ability to work collaboratively with multidisciplinary care team to develop and implement care plan.
  • Ability to communicate effectively with diverse audiences including professional staff, patients, community, insurance companies, etc.
  • Knowledge of patient teaching skills.
  • Demonstrated skill reflecting organization, self-motivation, initiative and fiscal responsibility, requiring little or no supervision to carry out duties.

Working Conditions

Works primarily in clinical setting, including clinical/patient care areas and office or meeting areas. Most work will be performed in a modern, well lit, air-conditioned area. Primarily daytime hours with flexibility required to meet needs of patients. Ability to utilize general office equipment including but not limited to telephone, computer, printer, copier, fax machine, as well as general clinical equipment such as an EKG machine, thermometer, weight scale, etc.

Considerable walking, standing, reaching, stooping, bending, twisting, pushing, pulling, and lifting (occasionally in excess of twenty-five (25) pounds). Mental effort required to assess, plan, and make or recommend decisions in complex environment. Likely to be exposed to blood/body fluid, including infectious diseases.
