Full Time, 8 HR Day Shift, Monday-Thursday 8-5;
Friday 8-12

Position Summary

Collaborates with physicians, nurses, and other healthcare providers to optimize patient outcomes and achieve mutual goals. Empowers patients to work as a partner with the healthcare team through education on understanding medical procedures and terms, reducing time constraints on physicians. Responsible for identifying patient/family needs and for coordinating supportive care and appropriate services in accordance with the interdisciplinary plan of care.

Promotes the delivery of quality patient care to patients with cardiovascular disease through implementation of the nursing process for the assigned patient population. Serves as a resource for nurses and other healthcare providers. Monitors patient outcomes and resolves issues/problems that may develop in the implementation of care. Maintains an atmosphere which ensures SGMC’s values, goals, policies, and procedures are consistently supported and enforced.

Education
  • Current, unencumbered license as a Registered Nurse in the state of Georgia required.
  • Bachelor of Science in Nursing preferred.
Experience
  • Minimum of two years nursing experience required.
  • Experience with cardiac patient population strongly preferred.
Knowledge, Skills, and Abilities
  • Skilled at setting priorities and utilizing problem solving techniques.
  • Ability to recognize and analyze regulations, standards, clinical issues, and patient outcomes.
  • Excellent verbal and written communication skills required. Excellent computer skills required.
  • Ability to operate a variety of office, educational, clinical and non-clinical equipment.
  • Demonstrated use of clinical skills and expertise in the area of orthopedics.
  • Ability to comprehensively assess patient’s physical and psychosocial status.
  • Ability to effectively and efficiently coordinate, implement and monitor the care management process and treatment plan for each patient.
  • Ability to communicate openly and empathetically with patients and families.
  • Ability to work collaboratively with multidisciplinary care team to develop and implement care plan.
  • Ability to develop and implement in-house education and community outreach programs, workshops, seminars, and other support services.
  • Ability to communicate effectively with diverse audiences including professional staff, patients, community, media, and other service agencies.
  • Knowledge of adult learning competencies and patient teaching skills.
  • Demonstrated management skills reflecting self-motivation, initiative and fiscal responsibility.
Working Conditions – ADA Information

Works primarily in clinic setting, including clinical/patient care areas and office or meeting areas. Primarily daytime hours with flexibility required in work schedule, assignments, and workload volume to meet the needs of the patients and clinic. Considerable walking, standing, stooping, bending, lifting (occasionally in excess of twenty-five (25) pounds). Focused mental effort required to assess, plan, and make or recommend decisions in a complex environment.

Likely to be exposed to blood/body fluid, including infectious diseases.
